Lending Materials Request

Please read the information at the beginning of the form. Then fill out the form to request one or more of our outreach trunks, learning kits, or DVDs for your class. Borrowers are responsible for shipping costs of all lending materials. The materials you request will come with an invoice and details about return shipping. All lending materials are sent UPS Ground.
